Nais simplex is a delicate, small worm with a brownish coloration. It has a bluntly triangular prostomium adorned with hairs, and eyes are absent. Dorsal setae originate from segment VI, each bundle containing 1 hair and 1 needle. The hairs are long, simple, nearly straight, and smooth. Needles are very short, simple-pointed, and fairly obtuse, distal nodulus. Both hairs and needles are absent in the last few posterior segments. Ventral setae start in segment II, with 2-3 per bundle measuring 88-135 µm long. The ventral setae of this segment are thinner than those in the succeeding segments, with those of the last segment measuring 88-90 µm. Ventral setae are bifid, with the distal tooth thinner and slightly longer than the proximal one. Budding was not observed. The pharynx is located in segments II-III, the esophagus is thin and found in segments IV-VII, and the stomach is in segments VIII-IX. The anus is postero-dorsal, and the blood is yellowish.
